Incidents and Emergencies

Incident Reporting

It is important that all Environment, Health and Safety incidents are reported promptly to your supervisor via the electronic Themis Self Service incident reporting system.

Further information about incident reporting is available from the EHS Unit's Incident Reporting webpage, at: www.pb.unimelb.edu.au/ehs/ehs/incidentreporting/

To report an incident, log in to Themis and select "Create new incident report" from "Staff Self Service" menu.

Staff, students, contractors, visitors and others without access to Themis may continue to use the revised hard copy Incident Report (New S3 Form)

Emergency Information for Buildings

Please familiarize yourself with all the fire exits of the buildings where you work/visit. You should equally make yourself familiar with the location of fire extinguishers in your immediate work area and areas you frequent so that you may be better prepared should an incident arise requiring you to use one. Refer to the Emergency Personnel List located on staff notice boards for emergency personnel and First Aid Officers in your building. "Emergency" posters and floor plans are posted in the main corridors of all buildings.
